The 32-year-old had his lost his son, Gehrig John Neshek, not long after tweeting updates about his son's delivery.
Neshek flew to Oakland from Florida after his baseball team, the Oakland Athletics captured the AL West title, to witness his son's birth.
Neshek went straight to hospital after defeating the Texas Rangers in the team's first playoff game.
When he arrived in Florida, Pat and his wife Stephanee Neshek both used the hashtag '#playoffbaby' as they tweeted play-by-play-like reports of the baby's impending arrival.
"Thinking of the long nights in AAA, my TJ surgery & the days when I thought it was over..Makes this the sweetest playoff birth of my career!" he tweeted Oct. 1.
On Oct. 2 he announced his wife was in labor: "What a day! My wife is now in labor!"
On Wednesday, Oct. 3 he announced the baby's arrival with a photo attached.
"It's a boy Gehrig John Neshek...8 lbs 5 oz, born 10/2/12. Pat flew Oakland to FL just in time for birth! #playoffbaby"
In the photo, baby Gehrig is wrapped in a hospital blanket while cradled in his mother's arms as she is lying in the hospital bed. The baby's eyes are open as his father is standing next to them beaming with pride.
However, shortly after midnight on Oct. 4, the couple delivered the somber news of the loss of their newborn.
"I am still in complete shock over the loss of our son, after he was on this earth for less than 24 hrs. He was perfect in every way and I loved him so much!" Stephanee tweeted.
"I am at a loss for words right now... he died peacefully sleeping in my arms unexpectedly without any explanation."
"There was no cause of death and I am having trouble understanding why God can let such a thing happen an innocent being," she said. "Every kind word helps."
Stephanee followed up her announcements by thanking their fans who expressed their condolences on the social media site.
