More details continue to emerge on the tragic death of Oscar winner Philip Seymour Hoffman, who was found dead in his Greenwich Village apartment Sunday from a reported drug overdose.

The 47-year-old actor was found dead on the bathroom floor of his apartment with a needle sticking out of his arm, The Daily Mail reports, shedding light on the actor's apparent recent relapse into addiction, a fact his former partner, Mimi O'Donnell has allegedly confirmed to police.

O'Donnell has reportedly told NYPD investigators that she last saw Hoffman around 2 p.m. Saturday, and that he appeared to be under the influence of drugs at the time. She also told them that when she spoke to him again around 8 p.m. via the phone, he also sounded high.

It was O'Donnell who began to worry about her former partner Sunday when he failed to show up at the $4.4 million apartment they shared on Jane Street to pick up their son Cooper, 10, and daughters Tallulah, 7, and Willa, 5. She reportedly asked close friend and playwright David Bar Katz and assistant Isabella Wing-Davey to check on Hoffman.

The two found him in the bathroom and called 911, but Hoffman was pronounced dead at the scene.

The scene investigators have reportedly found included the discovery of 49 containers of heroin, nearly two dozen discarded syringes, and anti-anxiety prescriptions which Hoffman didn't appear to have a script for.

Investigators also reportedly found eight empty bags stamped with the logo 'Ace of Spades' and 'Ace of Hearts'-a lethal mix of heroin laced with fentanyl-which has been linked to more than 100 deaths in the United States, including 20 in Pennsylvania this month, and 22 in Rhode Island within the first two weeks of the year.

The discovery has prompted questions about whether Hoffman spiraled out of control with his addiction following his reported split from O'Donnell in recent months.

It is unclear when the two allegedly split, though Hoffman reportedly began renting his $10,000-a-month apartment in Bethune Street in Greenwich Village in October.

Hoffman's last public appearance was at the 2014 Sundance Film festival in January, where he promoted his movies, God's Pocket and A Most Wanted Man.

He won an Oscar in 2006 for his portrayal of Truman Capote in Capote.
